Client Requirements

Contact us at 248-437-9790 to set up an appointment.

Items to bring to your appointment:

  • New clients please bring with you Social Security cards for every family member.
  • Current photo ID - Michigan ID card or drivers license for all family members 18 years of age and older in the home
  • Department of Health and Human Services award benefit letter
  • Proof of all household income for family members 18 years of age and older (pay stub, unemployment award letter, child support, bank statement, etc). You may use an income tax return, or other legal document.

Identification Cards in Michigan

You must apply for a non-driver ID card in person at a Michigan Secretary of State office. You will need to provide a number of documents and pay the required fee. Renewals and replacements for standard ID cards may be completed either online, by mail, or in person.  You must be a US citizen and US resident to obtain a card. Case Management

Active Faith works through a case management program, offering individuals and families:

  • food
  • clothing
  • children's tutoring
  • THAW Fund site for utility help

And Remember, even if you do not fit into our financial guidelines, we are an information and referral center for food, clothing, and utility help from other agencies.

FOOD PANTRY

If you are in need of food, please give us a call at 248-437-9790. You do not need to live in South Lyon to receive food if you are in need. We will help you find a food pantry in your area if you don't qualify with us. If you wish to become an Active Faith client to shop in the food pantry, please call for an appointment.

We are open from 10:00 a..m. to 5:00 p.m., Monday, Wednesday and Thursday. If you are in need of food and we are not open, please call 211.

CLOTHING

Any "gently used, clean" clothing can be dropped off at the east side of our warehouse located at 401 S. Washington Street in South Lyon. Drop off times are on Wednesdays only from 10:00 a.m. to 3:00 p.m. We are currently accepting Fall clothing items as well as clean blankets, sheets, and towels. Collected items from donations are distributed on the second and fourth Monday of the month and in emergencies. Please contact us at 248-437-9790, if you are in need of emergency clothing items.

Any items other than clothing, please call Purple Heart at 734-728-4560 or Church of Christ at 810-229-7051. ITEMS LEFT OUTSIDE AFTER HOURS WILL BE DISCARDED. Any questions please call 248-437-9790.

TUTORING

At times children struggle with basic math and reading skills. Contact us if your child needs assistance in one of these areas. You must be a current Active Faith client with a child enrolled in one of the South Lyon Community Schools elementaries. Active Faith Community Services office hours are between 10:00 a.m and 5:00 p.m., Monday, Wednesday and Thursday. You can also contact us by email on our Contact Us page.
